      <image:caption>Hypertension is a condition in which there has been persistently increased force of circulating blood against the walls of the arteries.  Blood pressure readings are recorded as two numbers – systolic (the top number) and diastolic (the bottom number).  They indicate the force of the blood against your arteries when the heart is beating (systolic) and when it is at rest/between beats (diastolic).   Anything under 120/80 is considered a normal blood pressure.   Hypertension is often called the “silent killer” because most people experience no symptoms of high blood pressure, therefore annual physical exams are very important in maintaining good health and well-being.  Taking blood pressure medication resulting in a well-controlled/normal blood pressure does not remove your diagnosis of hypertension, but will reduce your risk of complications.</image:caption>

      <image:caption>In diabetes, the body’s ability to produce insulin is impaired.  Insulin is the hormone that allows your body to use stored glucose for energy.  Pre-diabetes is characterized by higher-than-normal blood glucose levels, but not high enough to be diagnosed with diabetes; usually with a hemoglobin A1c between 5.7-6.4%.  Almost 50% of adults 65 years of age and older in the U.S.  have pre-diabetes.  Early identification and lifestyle changes are instrumental in reducing the risk of progression to type 2 diabetes.   Diabetes has a significant impact on multiple organs and systems, and the kidneys and retinas are particularly prone to damage.   Diabetes is the leading cause of blindness in the U.S. and annual eye exams are necessary to evaluated the retinas for damage.  Diabetes is also the primary cause for kidney failure in the U.S.  Kidney damage is signaled by protein, or albumin, being excreted in the urine, and also in elevated blood pressure.</image:caption>

      <image:caption>Hyperlipidemia is a condition characterized by too many fatty substances in the bloodstream which increase one’s risks for atherosclerosis, or a build up of fatty plaques on the walls of the arteries which can block the natural flow of blood.  Lipids are commonly broken down into cholesterol, triglycerides, high-density lipoproteins (HDL) and low-density lipoproteins (LDL).   HDL, or the “good” cholesterol, absorbs cholesterol and transports it back to the liver where it can be flushed from the body.  Studies have shown that high levels of HDL can lower your risk for heart attack and stroke.   LDL, or the “bad” cholesterol, makes up most of your body’s cholesterol.  It is harmful because it can build up on the walls of the arteries in the form of “plaque”.  Over time, these plaques can build up and cause narrowing of the vessels and restricted blood flow to vital organs leading to damage.  This significantly increases your risk of heart attack and stroke.   Self-care for those with a diagnosis of hyperlipidemia include a heart-healthy diet, physical activity, weight control, smoking cessation and medication adherence.   A heart-healthy diet should have a focus on whole grains, fish, fresh fruit and vegetables and should include 20-30 grams of fiber daily.  Saturated fats (foods where the fats are solid at room temperature; mostly found in animal fats) should contribute less than 7% of your daily calories.    Total fats should contribute 25-35% of you daily calories.  One should focus on less than 200 mg of cholesterol per day.  Absolutely, one should avoid trans fats (those that are used in prepared foods where liquid oils are turned to solid fats like shortening or margarine; anything fried and/or battered will fall in this category); these are the unhealthiest fats of all.</image:caption>

      <image:caption>When you breathe, your lungs take in oxygen from the air and delivers it to the bloodstream to ensure your organs and body tissues receive what they need to survive. The cells in your body need oxygen to work and to reproduce. During a normal day, the average adult breathes more than 20,000 time per day. Millions of Americans have lung disease, and it is the number 3 killer in the United States today. COPD (Chronic Obstructive Pulmonary Disease) is a chronic lung disease which leads to obstructed airflow, making it difficult to breathe. Emphysema and chronic bronchitis are the most common conditions that lead to COPD. With COPD, it becomes increasingly difficult to get vital oxygen into your lungs, and move toxic carbon dioxide out of your lungs leaving a lasting effect that cannot be reversed. The use of bronchodilators and corticosteroids can help to reduce symptoms and minimize further damage to the lungs. Asthma is a chronic lung disease which causes the airway to become inflamed making it difficult to breathe. Physical activity and exposure to environmental triggers such as chemical irritants and allergens can provoke an asthma attack, sometimes severe enough to require emergency treatment to prevent mortality. The uses of short-acting medications can quickly reduce the inflammation in the airway to make breathing less difficult and long-acting medications can reduce your risk of an asthma attack. Sleep Apnea is not uncommon, and can potentially be a dangerous condition. While most people are alerted to sleep apnea by a partner’s complaints of snoring, it is much greater than the nuisance of sleep disruption. Sleep apnea can cause daytime fatigue, headaches and interrupt your concentration, but it can lead to hypertension, heart attack, stroke, and other detrimental health problems. Sleep apnea can be obstructive, when the soft tissues in the neck collapse due to muscle relaxation during sleep; central, when there is a miscommunication between your brain and diaphragm, the muscle that controls breathing; or complex, when you have both obstructive and central sleep apnea. Sleep apnea is generally treated with CPAP, BiPAP or AutoPAP devices. Pulmonary Hypertension differs from hypertension in that it is specific to an abnormally increased pressure in the vessels between the lung and the heart. This results in the vessels becoming damaged, hardened and/or narrowed and which creates more work on the heart to be able to pump blood through your lungs. There are five types of pulmonary hypertension, and the treatment is dependent on which type one has. Diagnosing pulmonary hypertension requires that other diseases such as COPD and sleep apnea are ruled out.</image:caption>
